    A document required by law (the so-called "Lex Kamilek") for organisations working with children — it describes the rules for the safety and protection of minors at the studio.

Electronic document signature

Fill in the form to sign the consent electronically. After you submit it, we'll generate a PDF document with your signature and the date, and send it to the studio's address and your email.
